Though he had initially planned to play for the team in his sophomore season, he later changed his mind and declared himself for the NBA draft. He was drafted as the fourth overall pick in the 2011 NBA draft. This made him the highest drafted Canadian born player in history.

He made his professional debut in a match against his hometown team Toronto Raptors. In just seventeen minutes, he scored 12 points and 5 rebounds. His performance grew better in the next few games. He finished the season with averages of 8.2 points and 6.5 rebounds. He also became the first ever Canadian to earn All-Rookie Team Honors in the NBA.

In the 2013, off-season, he made the decision to switch his shooting hand to right rather than his left. Despite this change, his performance was almost the same in the 2013-14, season as it had been in the previous one.

During the next season, he helped the Cavaliers reach the 2015 NBA finals. They faced the Golden State Warriors, and were defeated in six games in the series. After the season, Thompson became a restricted free agent.
In October 2015, he signed a contract worth $82 million with the Cavaliers. He helped them reach the NBA finals once again, where they made NBA history by coming back from a 3-1 deficit in a game against the Golden State Warriors. They eventually won the series in seven games and earned the championship.
In December 2016, he became the first player in the history of the franchise to play in 400 games consecutively. Though his team reached the NBA finals for the third time, they were eventually beaten by the Golden State Warriors.
