Mahanati is a biographical drama film which revolves around Savitri, a popular actress in the South Indian film industry, who was immensely popular and successful for two decades. However, due to tragic incidents in her personal life, she faces depression, which makes her an alcoholic.

Rama Raju is an Indian cop working for the British government during the time of Colonial Rule in India. Despite his loyalty to the British government, he never receives his duty. He is soon sent after a tribal revolutionary named Komaram Bheem, but eventually both men join forces to fight for their country’s freedom against the British oppressors.

Raghuveer is in love with Indira, but he soon comes to know that he had desired her in his previous life as well, but he was killed by her lover, a warrior. He soon discovers that a young man, Harsha, who is also in love with Indira, is the reincarnation of the warrior who had killed him in his previous life. Satya and Deepthi decide to escape from their city, as Deepthi is being pursued by a gangster named Veeru Bhai, who wants her for himself. However, their bus is stopped unexpectedly by the gangster’s men, who kills Deepthi and fatally wounds Satya. Satya is however saved by Dr Sailaja, who also does a face transplant on him, giving him a completely new look. However, his new face begins to create problems for him. He feels that Dr Sailaja may be hiding something.
